On February 6, during the joint panel discussion at the Second Session of the 13th Shijiazhuang Municipal Committee of the Chinese People’s Political Consultative Conference, Han Shuqin, a member of the CPPCC and Deputy General Manager of the company, put forward proposals on how to leverage medical consortia as a key lever to effectively implement tiered diagnosis and treatment, mobilize the enthusiasm of medical resources at all levels, and alleviate the widespread public concern over the difficulties and high costs of accessing healthcare. She emphasized the need to strengthen tiered diagnosis and treatment, invigorate medical resources at all levels, and advance the construction of “Healthy Shijiazhuang.”

“As healthcare reform enters the ‘deep-water phase,’ the situation in which large medical institutions are overcrowded while grassroots medical facilities remain largely underutilized has yet to be fundamentally improved, and numerous challenges still need to be addressed.”

“It is essential to design robust systems for tiered diagnosis and treatment and for medical insurance payment that balance the interests of all stakeholders, clearly define the responsibilities of each party, and ensure that tiered care remains cohesive rather than fragmented,” proposed Committee Member Han Shuqin. She recommended strengthening policies to promote the development of closely integrated healthcare consortia, refining institutional frameworks for revenue allocation and cost management among such consortia, leveraging the regulatory power of medical insurance policies to appropriately widen price differentials across healthcare institutions at different levels, promptly and scientifically adjusting the design of the medical insurance system and reimbursement policies, increasing the degree of differentiation in reimbursement rates based on the level of care, establishing and improving policy guidance that encourages insured individuals to seek initial consultation at primary-level facilities, and tilting resources toward primary-care institutions and patients referred in accordance with established protocols, thereby fostering orderly patient flow and access to care.

“To implement tiered diagnosis and treatment and keep patients at the primary-care level, primary-care institutions must have the capacity to effectively manage patient care. The key is to build a high-caliber workforce for primary care, streamline talent-mobility mechanisms, and create more opportunities for the professional development of young physicians, so that they are both retained and fully utilized at the primary level.”

“To ensure that tiered diagnosis and treatment truly delivers results, we must address the pressing practical challenges it currently faces, such as the lack of unified referral standards and robust oversight mechanisms, so that patients can both ‘move up’ to higher-level care when needed and ‘move down’ to lower-level care when appropriate.” She argues that medical consortia should establish smooth, streamlined two-way referral pathways, refine the criteria for such referrals, and put in place corresponding performance evaluation, reward-and-penalty, and oversight systems, thereby providing clear guidelines for healthcare institutions and physicians at all levels as they advance tiered diagnosis and treatment. She further recommends establishing an Internet-plus information support system for medical consortia to facilitate the effective deployment of high-quality medical resources to lower-level facilities, thus fully leveraging the critical role of technology in advancing tiered diagnosis and treatment.
